State in which false entry, rewriting, erasure, and confusion are prevented for records created with legitimate authority and also it is clear as to where the responsibility for creation lies from the perspective of any third party. In this case, confusion means that a record is made by mistaking one patient for another, or that the recorded information is incorrectly related to the other.
